The Vermont Program for Quality Health Care (VPQHC) launched their Connectivity Care Package Pilot Program (CCP) this week. which will use Coronavirus Relief Funds (CRF) to provide health care practices and facilities with digital equipment and training to help connect their patients to telehealth services. The CCP program aims to provide equitable access to digital equipment to those Vermonters who need it and targetted to patients who have experienced a disruption in in-person care due to COVID-19. Eligible practices will receive free Apple ipads to distribute to their patients who need them for telehealth purposes, along with digital literacy resources and wifi boosters if necessary. The deadline to apply for the program is Monday, October 5th, 2020. This application window is extremely brief because the CRF funding requires a very quick turnaround. If you have patients you believe would be eligible for this program please email ccp@vpqhc.org. In fully completing the application, VPQHC can get an understanding of the on-the-ground-reality of the need for telehealth services and internet connectivity to provide the right equipment for your patients in an expeditious manner. For the application please email ccp@vpqhc.org.
